Keeping it under control rests on taking medication every day, measuring blood pressure regularly and following lifestyle and dietary rules that many elderly or working patients struggle to maintain on their own. Measuring blood pressure at home, repeated in the calm of everyday life, gives a far more reliable picture than one-off readings at the surgery and guides a more accurate adjustment of treatment.
What the nurse does at every visit
Blood pressure measured at rest following a standardised protocol (several readings, suitably sized cuff), pulse check, verification that medication is being taken and a search for side effects such as dizziness or oedema.
Visit frequency and monitoring
2 to 3 readings per week for as long as blood pressure is not yet stable, then a weekly reading for maintenance. The values are recorded on a blood pressure log handed to the physician.
Coordination with your treating physician
The blood pressure log and any reported symptoms are passed on to your treating physician or cardiologist. Any change of treatment remains their decision; the nurse applies it and monitors.
Equipment and home monitoring
An approved, calibrated electronic blood pressure monitor, a cuff suited to the patient's build, monitoring of weight and of swelling in the lower limbs. The nurse also checks adherence to the low-salt diet.
The family's role between visits
We teach relatives to recognise warning signs (severe headaches, visual disturbances, chest pain), to limit salt and to make sure the daily medication is taken.
AMO / CNSS coverage
Chronic hypertension falls under long-term illnesses (ALD). Nursing acts (AMI) are coded according to the NGAP and reimbursable by AMO or CNSS, with a compliant coded receipt and a signed care record at every visit.
